If I am using the keyboard while the rest break warning displays, it no longer (since 1.8.6) goes to the rest break but instead automatically postpones.  It repeats this behavior at the next rest break warning, and then on the third one it actually does go to the rest break.

Correct. 

This can be controlled by the 'maximum number of prompts' settings in the preferences. A break is forced after 'maximum number of prompts' times. You can set it to 1 if you want to be forced to take a number after the first prompt.
