The following situation occurs with Win7 32bit: If I connect my mouse directly via USB to my laptop and use the integrated keyboard, Workrave works correctly. When setting up the laptop at me office, using an external screen, plugging the mouse and an external keyboard to the USB hub in that screen, not all activity is noticed. Breaks happen much more rarely, most times when I look at Workrave it's still close to the beginning of the countdown. Is this behaviour known? Tested with 1.9.4 and 1.9.3.

Strange... Could you try the latest snapshot: http://snapshots.workrave.org/20111022/workrave-win32-v1_9_900-4-g20b4efeafa-20111022-installer.exe This version uses a different way of monitoring user activity.

It's very hard to test, as there is another bug preventing me from really using it: On the notebook computer, quite often I wonder why I don't see any Workrave popups, just to find out that Workrave is in suspended mode. I never suspended it manually, I guess it goes to suspended mode when the computer hibernates or goes to standby and doesn't come back correctly.
